Two options for membership:
(1) Monthly - must be ACH (automatic withdrawal)
(1) Check for 1st monthly amount (min $20)
(2) ACH Form Completed
(3) Membership Form Completed
(2) Annual - must be prepayed amount for a full year
(1) Check for the full year of membership (min $240)
(2) Membership Form Completed
